Overview
Diagnosis is the process of identifying the disease, disorder, or condition responsible for a patient's signs and symptoms, integrating clinical history, physical examination, laboratory testing, imaging, and increasingly molecular and genomic analysis. It rests on the systematic interpretation of evidence: generating a differential, applying diagnostic criteria, and weighing test characteristics such as sensitivity, specificity, and predictive value to confirm or exclude candidate conditions. Modern diagnostics span laboratory assays, histopathology and cytopathology, functional and cross-sectional imaging, and the detection of biomarkers and pathogen-specific or somatic mutations. Accurate and timely diagnosis determines prognosis and guides therapy, while diagnostic error, delay, and disparity remain important sources of avoidable harm. The field continually refines its tools to distinguish overlapping presentations, characterise rare entities, and personalise management.
