Description of Special Issue

Primary care is the principal setting where prevention and chronic disease management can be delivered at scale, yet systems continue to face persistent inequities in access, quality, and outcomes. At the same time, primary care practices are experiencing workforce strain, rising administrative burden, and increasing burnout, threatening the sustainability of essential preventive and chronic disease services.


Recent shifts in care delivery—including team-based models, value-based payment structures, digital health tools, remote monitoring, and population-health workflows—have created new opportunities for improving outcomes. However, the evidence base remains fragmented across settings and often lacks implementation clarity, equity-centered design, and workforce implications.


This Special Issue aims to address these gaps at the intersection of primary care redesign, implementation science, health equity, and workforce sustainability. We invite scientifically rigorous and implementation-oriented contributions that provide actionable insights for clinicians, educators, health system leaders, and policymakers across diverse primary care environments.

  • Equity-centered models addressing disparities in outcomes, access, and care experience
